The Goal

Identify patients diagnosed with a particular rare disease who had stopped therapy.

This group is commonly known as lost to follow-up (LTF).

The Impact

Across 172 accounts, health systems ran an EHR report to identify patients who had fallen out of care. In just minutes, those efforts surfaced 51 patients who were LTF. These individuals were at risk of disease progression without ongoing therapy.
