Description: Most payer institutions employ an oncology pathway that directs prescribers to preferred treatment options for their cancer patients. Integrated Delivery Networks, and provider health systems may use their own customized pathway while many others rely on NCCN, as a peer-reviewed evidence based compendia to support drug selection. The evidence used in these pathways serves as the foundation for supporting policy and access. However, the environment is changing and NCCN recently clarified their position that same line of therapy step therapies are not clinically recommended. The session explores the influence of pathway organizations on access and how this will continue to change / adapt as scrutiny on utilization management programs persist.

Description:
Nearly every stakeholder agrees that traditional utilization management creates administrative burden, delays care, and generates provider frustration. At the same time, oncology costs continue to rise and payers remain accountable for managing spend. As CMS, states, and industry groups push for prior authorization reform, the question is no longer whether utilization management should change, it is what should replace it. The session will explore the next generation of oncology cost management: pathways, prospective clinical guidance, navigation-driven intervention models, AI-enabled decision support, and risk-bearing care management organizations. The discussion should move beyond criticism of prior authorization toward practical alternatives.

Title: Outcomes-Based Contracts in Oncology: Who Owns the Outcome?
Description: We’ve been talking about outcomes-based contracts in oncology for years. In reality, most haven’t scaled and some haven’t lasted. Payers want real accountability on high-cost therapies, especially when benefit is still evolving at launch. Manufacturers want access, but push back on endpoints they don’t fully control. Providers are being asked to track and report outcomes without getting paid to do it. This session gets into what’s actually worked and what hasn’t. Where have these contracts held up? Where have they quietly fallen apart? What endpoints are people actually willing to stand behind? And is real-world data usable enough to support this and how can we evolve the model for a win-win-win position? Outcomes-based agreements (OBAs) are contracts where payment or compensation is tied to the successful achievement of specific, predefined results rather than for time spent or services rendered. This model shifts financial risk from the buyer to the provider, ensuring accountability and aligning incentives.
