At the same time, traditional value-based agreements are evolving in response to new definitions of value, care delivery shifts, and ongoing cost pressures. The IRA's introduction of spending caps for Medicare Part D beneficiaries in the catastrophic phase has dramatically reduced out-of-pocket costs for people with cancer from potentially thousands of dollars to a maximum of $3,300 in 2024 and $2,000 in 2025. This shift is a lifeline for patients facing exorbitant costs for drugs like Olaparib and Imbruvica, which can exceed $150,000 annually. However, it also raises critical questions about the broader financial implications for payers and providers. Critics argue that lower out-of-pocket costs may lead to higher premiums and increased demand for costly medications, while pharmaceutical manufacturers might raise prices on new drugs to maintain profit margins.
